At District 360, we understand that each individual has unique needs and goals, so our support services are designed to assist participants with everyday activities, ensuring they can live as independently as possible. We offer personal care, with our compassionate team providing services such as showering, dressing and grooming, always maintaining dignity and respect. Additionally, we provide domestic assistance with household tasks like cleaning, cooking, and laundry, allowing you to focus on your well-being and enjoy a comfortable living environment. Our team also assists with meal planning and preparation to ensure you have nutritious meals that cater to your dietary needs and preferences. Furthermore, our transportation services ensure you can attend medical appointments, community events, and other important engagements without hassle.

Support Work Services
- Assistance with Social, Economic & Community Participation

At District 360, we specialize in providing a wide range of support work services designed to assist participants in social, economic, and community participation. Our services include assistance with social participation by supporting relationship building, joining social groups and activities, and encouraging participation in community events and programs. We offer economic participation guidance on employment opportunities, job readiness, vocational training, skill development, and accessing financial resources and planning. For community participation, we help with integrating into community activities, local events, volunteering opportunities, and engaging with community services and resources. Our team is committed to providing personalized services tailored to meet the unique needs of each participant, empowering individuals to live their best lives by fostering independence and active participation in their communities. Whether you need help with social activities, finding employment, or getting involved in local events, District 360 is here to support you every step of the way.

Supported Independing Living (SIL)

Our Supported Independent Living (SIL) are available 24/7 to provide you with the care needed for you to live as independently as possible. We can assist if you are looking for assistance in a shared home environment or even for assistance in your own individual home. We believe strongly in drawing up a plan which is tailored to your personal needs and preferences, as we work hand in hand with our participants and the NDIS to ensure that we select the right team of support workers for you.

Social Group Programs (Perth only)

Our social group programs at the District 360 Community Hub in East Perth bring people together to learn, create, and enjoy each other's company. We offer a variety of workshops and weekly group programs tailored to different interests and skills, including a peer-facilitated arts and crafts group program, a painting group program, and a well-being group program designed to promote relaxation and mindfulness. Throughout the year, we provide unique special programs such as cake decorating, terrarium making, and up-cycling workshops and programs. Additionally, we organize social group outings to events like AFL games, concerts, and other local happenings around the Perth area, offering excellent opportunities to enjoy community events and build lasting friendships. Group STA Respites
(Short-Term Accommodation)

Every summer, District 360 offers an unforgettable experience with our group Short-Term Accommodation (STA) respites to Rottnest Island (Wadjmeup) at the beautiful Discovery Resort Rottnest Island, where participants enjoy a unique glamping experience. The three-day, two-night respites are charged to the CORE budget of a participant's NDIS plan. During the stay, participants engage in various activities designed to build capacity and foster friendships, including art sessions by the beach, swimming in the resort pool or ocean, group BBQ cooking, bike rides, and guided bus tours to explore the island's beauty and history. Safety and comfort are our top priorities, with our dedicated staff present throughout the respite to ensure support. The glamping tents are shared between two people, providing a cozy, friendly atmosphere. Join us for a memorable and enriching experience at Rottnest Island this summer, build your capacity, make new friends, and create lasting memories with District 360's group STA respites.

Customised STA Respites
(Short-Term Accommodation)

At District 360, we understand the importance of providing quality Short-Term Accommodation (STA) respite services tailored to each participant's unique NDIS goals. Our customized STA respites are designed to offer a well-deserved break for regular carers while ensuring that participants receive the support and care they need in a welcoming environment. We take the time to understand each individual's specific needs, interests, and passions to create meaningful and engaging experiences. Whether it's pursuing a favourite hobby, exploring new activities, or simply enjoying a change of scenery, our customised STA respites are crafted to enrich the lives of our participants while providing peace of mind to their carers.

Support Coordination

At District 360, we specialise in offering a wide range of support coordination services to help you get the most out of your NDIS package. Our range includes Lvl 2 Coordination of Supports, Lvl 3 Specialist Support Coordination. Our experienced team of coordinators work hard to ensure that your reports are accurate and timely and can connect you with the right services that are tailored to your individual support requirements. They strive to help you live independently and enjoy a better quality of life.

Counselling

District 360 has taken the next step of our NDIS registration and have applied to add 0128 Theraputic supports and so we can now provide counselling services to self-managed and plan managed participants. Our counsellors are passionate professionals who are dedicated to listening and taking care of every individual. They construct personal plans that work best for each participant in order to fulfil their goals and enjoy life’s challenges. Regular reports with these plans ensure that each participant receives involved care and updated advice on how to take charge of their mental health. We believe our counsellors are the key to making your NDIS experience uplifting; in our hands, you can trust that any challenge can be overcome.

Psycho-social Recovery Coaching (PRC)

At District 360, our psycho-social recovery coaching services form an integral part of our holistic approach to NDIS support. Managed by our dedicated support coordination team, these services are designed to empower individuals with psycho-social disabilities to achieve their personal goals and improve their well-being. Our expert coaches work closely with participants to develop tailored recovery plans, build resilience, and enhance life skills. By providing continuous guidance and practical support, we help our clients navigate the complexities of daily life, fostering independence and promoting a positive outlook. Whether it's accessing community resources, managing mental health, or developing social connections, District 360 is committed to making a meaningful difference in the lives of those we serve.
