About the Role
As a Disability Support Worker at District 360, you will play a pivotal role in providing high-quality care and support to individuals with disabilities. Your primary responsibilities will include assisting clients with daily living activities, promoting their independence, and enhancing their quality of life. You'll work closely with a dedicated team to develop and implement personalised care plans tailored to each client's unique needs. This role requires compassion, patience, and a strong commitment to advocating for the rights and well-being of individuals with disabilities. Requirements
Key Responsibilities
Client Support:
Assist clients in living the life they choose while maintaining confidentiality and keeping documentation current.
Documentation:
Enter end-of-shift notes into the District 360 mobile app system for accurate record-keeping.
Support Plan Implementation:
Follow and adhere to the tailored support plan designed for each client.
Incident Reporting:
Report any incidents or accidents immediately to key contacts and through District 360.
Communication:
Maintain open communication with support coordinators, managers, families, and other stakeholders about clients’ progress and relevant issues.
Program Implementation:
Develop, monitor, and implement client programs while training other staff as needed.
Consultation:
Facilitate communication between all parties involved in the client's day-to-day life.
Family Engagement:
Develop rapport and support contributions from family, guardians, or advocates to achieve positive outcomes for the client.
Report Preparation:
Complete end-of-period reports and reviews to assess client progress and formally update family and advocates.
Financial Arrangements:
Identify and document appropriate financial arrangements required for service users.
Health Monitoring:
Adequately record medical and health issues that arise and inform appropriate others when necessary.
Qualifications
Education:
Certificate III in a relevant field (desired) or equivalent work experience.
Certification:
Valid First Aid/CPR certificate (not older than 12 months).
Driving License:
A current driver's license with minimum insurance for a car not older than 10 years.
Physical Fitness:
Reasonable level of physical fitness, health, and resilience to fulfill the demands of the role.
